MUST be able to work as NYC local and open to receiving XEOMIN injections. Talent should have not received any facial neurotoxin treatments, including but not limited
to Botox®, Dysport®, or Juveau®, within three (3) months prior to the date.
XEOMIN is a prescription medicine:
• That is injected into muscles and used to improve the look of moderate to severe upper facial lines (frown lines, forehead lines, and crow’s feet) in adults for a short
period of time (temporary).
• That is injected into glands that make saliva and is used to treat long-lasting (chronic) drooling (sialorrhea) in adults and in children 2 years of age and older.
• That is injected into muscles and used to:
◦ treat increased muscle stiffness in the arm because of upper limb spasticity in adults.
◦ Treat increased muscle stiffness in the arm in children 2 to 17 years of age with upper limb spasticity, excluding spasticity caused by cerebral palsy.
◦ Treat the abnormal head position and neck pain with cervical dystonia (CD) in adults.
◦ Treat abnormal spasm of the eyelids (blepharospasm) in adults.They will be taking before and after photos and videos of you.

Usage/Run:
Use: Unlimited use in all mediums, excluding broadcast, only to advertise and promote Xeomin and to increase public awareness of its benefits as an aesthetic medical procedure.
Territory: Worldwide
Term: Five-years from the first commercial use

-consent (not open to) to aesthetic medical treatment with Xeomin, as they will get injected 10-14 days prior to the after shoot
-recognize and acknowledge that my treatment with Xeomin will involve alterations to my facial aesthetic appearance and may carry a risk of personal injury.
-have not received any facial neurotoxin treatments, including but not limited to Botox®, Dysport®, or Juveau®, within three (3) months prior to the date of signing this Agreement. Additionally, I confirm that I have not received Daxxify® in my face within six (6) months prior to the date of signing this Agreement.
